Important: Under medical device reporting regulations put forth by the FDA, an initial importer is required to report incidents in which a device may have caused or contributed to a death or serious injury as well as certain malfunctions. Please indicate if you are reporting an adverse event for a product manufactured by a foreign manufacturer that is represented by Dawa Medical as their initial importer.
